I'm Spencer Goff, a Doctor of Physical Therapy specializing in neurological rehabilitation. I work with stroke and brain injury patients at every stage of recovery — from those just leaving inpatient rehab to people years post-stroke who still want to make progress.

My practice is built around one belief: your brain can keep learning. And Neuro Rehabilitation is how we teach it.


Who I Work With

I work with adults recovering from Brain Injuries and:


  • Ischemic and hemorrhagic stroke
  • TIA (transient ischemic attack)
  • Stroke with resulting hemiplegia or hemiparesis
  • Stroke-related balance and walking difficulties
  • Stroke affecting arm and hand function

Therapy I use: Neuro-IFRAH Approach


I use
Neuro-IFRAH (Neuro-Integrative Functional Rehabilitation and Habilitation) — a specialized, hands-on approach developed specifically for neurological conditions like stroke and brain injury.


Unlike general physical therapy, Neuro-IFRAH focuses on:

  • Retraining the system, not just strengthening muscles
  • Restoring more normal movement to reduce compensations that limit long-term recovery
  • Working on the Whole Body — while taking Whole Person into account
  • Concentrated, movement-specific therapy to help neuroplasticity

This isn't passive therapy. Sessions are active, can be challenging, and tailored to where you are in recovery and where you want to go.


What Sessions Look Like

What to Expect in a Session

Every person's stroke is different, and your therapy should reflect that. In our first session, I'll do a thorough assessment of your movement, strength, coordination, balance, and functional goals.


From there, sessions typically focus on:

  • Gait and walking retraining — improving the quality and safety of how you move
  • Upper extremity function — working to restore arm, hand, and fine motor use
  • Balance training — building the confidence to move safely at home and in the community
  • Functional activities — practicing the real tasks that matter to you


I offer both
clinic-based sessions at my Solana Beach office and in-home therapy for patients who benefit from working in their own environment.
