For many stroke survivors, getting to a clinic isn't easy. Transportation, fatigue, mobility challenges, or simply the effort of leaving the house can turn every appointment into an ordeal — before therapy even begins.


In-home physical therapy removes that barrier entirely.


I'm Spencer Goff, a Doctor of Physical Therapy specializing in neurological rehabilitation. I bring the same self evidence-based, hands-on stroke rehab I provide in my Solana Beach clinic directly to your home — so we can focus on recovery, not logistics.


Why In-Home Therapy Is Often the Better Choice After Stroke

There's a strong clinical case for in-home Therapy — not just a practical one.

After a stroke, the brain learns best in familiar, meaningful environments. Practicing walking in your own hallway, reaching for items in your own kitchen, or navigating your own bathroom translates more directly to real independence than the same exercises done in a clinical setting.

In-home therapy also allows me to:

  • Assess your actual environment for fall risks, mobility barriers, and safety modifications
  • Tailor exercises to your real daily routine — not a generic program
  • Involve family members or caregivers naturally in the process
  • Observe how you actually move at home, which often reveals things a clinic visit won't

For patients in the early stages of recovery, or those who are medically fragile or have significant mobility limitations, in-home Therapy isn't just more convenient — it's often more effective.


What to Expect During an In-Home Session 


I arrive with everything needed for a full, productive session. You may not need any special equipment at home.


A typical in-home session may include:

  • Gait and walking practice in your hallway, driveway, or yard
  • Balance training throughout the space — the bathroom, kitchen, stairs
  • Upper extremity and hand function activites at your own table or workspace
  • Transfer training — getting in and out of bed, chairs, and the car safely
  • Caregiver instruction so your support system can reinforce progress between visits
  • Home safety assessment with practical recommendations to reduce fall risk


Sessions are active and progressive. I use the
Neuro-IFRAH approach — a specialized neurological rehabilitation approach focused on rehabilitation, not just building general strength.



Is In-Home PT Right for You?

In-home physical therapy after stroke is a strong fit if you:

  • Recently discharged from a hospital or inpatient rehab facility
  • Have difficulty with transportation due to mobility, fatigue, or caregiver availability
  • Are in the early stages of recovery and not yet safe to travel regularly
  • Live with significant weakness, balance issues, or use a wheelchair or walker
  • Want therapy that directly addresses independence in your own home
  • Are a long-term stroke survivor looking to continue making progress


You don't have to be homebound to benefit from in-home PT. Many of my patients simply find that the combination of convenience and environment-specific training produces better results.
